Ageing, Mental Health, and Healthcare Expenditure in Kerala: A Review of Current Trends and Implications

Introduction Kerala, the southernmost state of India, is widely recognized for its high human development indicators—literacy, life expectancy, and overall health outcomes. However, the state now faces emerging challenges associated with population ageing, increasing healthcare costs, and rising mental health concerns among older adults. The proportion of elderly individuals is projected to grow from 16.5% in 2021 to 26% by 2036, signalling a demographic transition with serious health and economic implications. Objectives This review synthesizes available evidence to examine Kerala’s demographic trends, assess the financial burden of healthcare, and explore the prevalence and implications of mental health conditions among older adults. Methodology The analysis is based on secondary data from the Longitudinal Ageing Study in India (LASI), National Sample Survey Organisation (NSSO) reports, and selected peer-reviewed studies focusing on ageing, healthcare expenditure, and mental health in Kerala. Critical Findings Kerala has the highest median age in India (35.12 years in 2021) and one of the highest old-age dependency ratios, increasing strain on the healthcare system. Out-of-pocket expenditure remains above the national average, with urban households spending ₹10,341 annually on hospital care and rural households ₹8,655. Mental health disorders affect about 11.36% of the population, with studies from Ernakulam and Thiruvananthapuram reporting elevated rates of depression and anxiety among institutionalized or socially isolated elderly individuals. Factors such as migration of younger family members, shrinking social networks, and limited community engagement further aggravate mental distress and healthcare dependence. Conclusion Kerala’s ageing population, coupled with rising medical and mental health burdens, demands urgent, integrated interventions.
