Background: Child marriage is both a public health concern and a violation of human rights, undermining individual growth and impeding national development. SDG 5.3.1 aims to eliminate child marriages by 2030, necessitating targeted micro-level studies. This research investigates the determinants, district-level patterns, and rural-urban inequalities of child marriage in Uttar Pradesh. Method: Utilizing data from the fifth round of the National Family Health Survey 2019-21, the study includes 20,358 women aged 20-24 (16,946 rural & 5,412 urban) in Uttar Pradesh. The chi-square test, logistic regression, and the Blinder-Oaxaca decomposition model was utilized in this study. We checked multicollinearity using the VIF and adjusted the analysis using survey set command for the survey’s complex design. Result: Child marriage prevalence in Uttar Pradesh was 15.8%, with a significant 8.2 percentage points difference between rural (17.9%) and urban (9.5%) areas. Districts like Shrawasti (51.9%), Lalitpur (42.5%), and Maharajganj (37.5%) exhibit the highest prevalence, while Bijnor (5.2%), Ambedkar Nagar (5.5%), and Saharanpur (7%) have the lowest rates. Even after adjusting for covariates, rural women face a higher risk of child marriage (OR=1.19; CI=1.03-1.38; p=0.013). The Blinder-Oaxaca model explained 88.3% of the rural-urban inequality, with education (38.8%), wealth index (28.2%), religion (16.6%), and mass media exposure (4.9%) being key drivers of inequality. Conclusion: Strategic interventions are imperative in high-prevalence districts like Shrawasti, Lalitpur, Bahraich, Balrampur, and Sidharth Nagar to combat child marriage and safeguard human rights. Policy measures should prioritize enhancements in education and wealth status.
